USDA Program Discrimination Complaint Procedure

If a person feels discriminated against by the Monroe County Child Nutrition Program, they have the option of filling out a USDA Program Discrimination Complaint Form (AD-3027). You or your authorized representative must sign the complaint form, OR write a letter addressed to USDA and provide in the letter all of the information requested in the AD-3027 form concerning your complaint to the USDA within 180 days of the occurrence of the incident.

You may obtain a complaint form by any of the following methods:

  1. Contacting Mr. Marty Hanks, Federal Programs Coordinator at 109 Pickens Avenue, Monroeville, Al 36460, office phone number: (251)743-2150 ext 1003, or email your request for the (AD-3027) form to MHanks@monroe.k12.al.us.

  2. Contact the Office of the Assistant Secretary for Civil Rights, at (202) 720-2600 or (800) 877-8339 (toll free) or send an email to the Office of the Assistant Secretary for Civil Rights atCR-INFO@ascr.usda.gov.

  3. Individuals who are deaf, hard of hearing, or have speech disabilities, may contact us through the Federal Relay Service on (800) 877-8339 or (800) 845-6136 (Spanish).

You are not required to use the complaint form. You may also file a program discrimination complaint by writing a letter to the Office of the Assistant Secretary for Civil Rights and provide in the letter all of the information requested in the form concerning your complaint to the USDA.
